What do I need to bring to my first appointment?
Your driver’s license, insurance card, and your prescription from your doctor if one was provided. We also recommend that you fill out the new patient forms that are sent a few days prior via email or text message and bring them with you to your first visit. If you do not do this, we ask that you arrive at least 20-30 minutes before your initial appointment to allow ample time for filling out the new patient intake forms.

Do you accept insurance?
